Common use
Conjugated estrogens are a hormone replacement therapy (HRT) used to treat symptoms of menopause, such as hot flashes, vaginal dryness, and osteoporosis prevention. They help replenish estrogen levels that naturally decline during menopause or after surgical removal of the ovaries. This medication is also used in some cases for female hypogonadism or certain types of cancer like advanced prostate or breast cancer. Conjugated estrogens work by binding to estrogen receptors, helping regulate various functions in the body.
Dosage and direction
The typical starting dose for menopausal symptoms is 0.3 to 0.625 mg daily, taken orally once a day, with or without food. Dosage may vary depending on the condition treated and patient response. For women with a uterus, progestins may also be prescribed to reduce the risk of endometrial cancer. Follow your doctor’s instructions exactly and do not stop the medication abruptly without medical advice.
Precautions
Long-term use of conjugated estrogens may increase the risk of blood clots, stroke, breast cancer, or dementia, particularly in postmenopausal women. It should be used at the lowest effective dose for the shortest duration consistent with treatment goals. Patients with a history of heart disease, liver disorders, or hormone-sensitive cancers should inform their doctor before starting therapy. Regular pelvic exams, breast exams, and mammograms are recommended during treatment.
Contraindications
This medication is contraindicated in women with undiagnosed abnormal vaginal bleeding, active or past breast cancer, estrogen-dependent tumors, blood clots, or liver disease. It should not be used during pregnancy or by women who are breastfeeding. Patients with a history of stroke or heart attack should not use estrogen therapy without a full risk assessment. Known allergy to estrogens or any components of the tablet is also a contraindication.
Possible side effect
Common side effects include headache, breast tenderness, nausea, abdominal pain, and vaginal bleeding or discharge. Some women may experience fluid retention, mood swings, or weight changes. Serious but rare side effects include blood clots, gallbladder disease, or high blood pressure. Seek immediate medical help for symptoms like leg swelling, chest pain, or vision changes.
Drug interaction
Conjugated estrogens may interact with thyroid medications, blood thinners (like warfarin), anticonvulsants, and certain antibiotics or antifungals. These interactions can alter hormone levels or affect the efficacy of either medication. Smoking may also increase the risk of cardiovascular side effects. Always inform your healthcare provider about all medications, supplements, and herbal products you use.
Missed dose
If you miss a dose, take it as soon as you remember. If it’s close to the next scheduled dose, skip the missed dose and resume your regular schedule. Do not double up to make up for a missed dose. Missing doses may reduce the effectiveness of the therapy.
Overdose
Overdose may cause symptoms such as nausea, vomiting, breast tenderness, drowsiness, or breakthrough bleeding. Severe overdose is rare but should be reported to a medical professional. Supportive care is typically all that is needed. Keep the medication out of reach of children to avoid accidental ingestion.
Storage
Store conjugated estrogens at room temperature (20°C to 25°C / 68°F to 77°F), away from moisture, heat, and light. Keep tablets in their original packaging until use. Do not store in the bathroom or near sinks. Dispose of expired or unused medication according to local guidelines.
